By clicking on the Edit button you are able to mark the question as required:
More questions can be added to the survey form by clicking on Add new:
Click on the text editor field available under Form Title to set the Title of the form.
Click on the text editor field available under the Form Success Message to set the Success Message of the form.
Select your preferred position to display the survey form on the Successful Order page.
Click on the checkbox option to enable the Form Submission notification and receive an email on form submission.
Enter the Email Address in the Email field.
Enter the Email Subject of your choice.
Enter the content in the Email Content Text Editor..
Click on the Save button to save the configured settings.
Click on the Preview button to preview the Survey Form which is created from our application.
